Serum Refill Request
Serum Refill Requests are Changing!
We hope this new process is easy to navigate and helps us centralize all communication through MyChart.
Please login to your MyChart account, or click the button below to login.
Instructions:
1. In the upper-left corner of your screen, click the Menu button (the icon with three lines.
2. Scroll down to the "My Record" section and select "Questionnaires".
3. From the list of available questionnaires, click on "Serum Refill Request".
4. Fill out the form and click submit.
Please allow up to two weeks for your serum refill to be processed.
